PET ADOPTION PRE-SCREENING QUESTIONNAIRE Thank you for your interest in adopting one of our homeless pets. To expedite the adoption process we have created this pre-screening questionnaire that will help us to find the best pet to match your family. Please bring a copy of this questionnaire and your responses to adoption day to show that you were pre-screened. After completing this form, a page will appear with your replies, save for your files and print this to assist us. This will make the process on our adoption day go much faster. Thank you! At this time we are only able to help place pets in Maricopa County, Arizona. Sorry, we do not have enough volunteers to help animals outside of the Arizona.
